 NMHS FOUNDATION SCHOLARSHIP APPLICATION
NMHS SENIORS ONLY



The application deadline is January 27, 4 P.M.
Late applications will not be accepted.

The process is not first-come, first-served.
See below for application and frequently asked questions.

 

Directions:

  1. Print a blank copy of the scholarship application to use as a working copy. Prepare your essay and future statement prior to completing the application online. Be sure your activity chart in CIS is complete.
  2. Identify two teachers from whom you want to request recommendations (page 4 of application). Give your resume/brag sheet to those two teachers. You may do this immediately; even before you complete your application. Teachers will receive the recommendation form and will submit recommendations directly to the College & Career Center via email. Complete the scholarship application online in its entirety. Print a copy for your records, and then submit the application and your CIS activity chart via email to: brandee.tolner@medford.k12.or.us - DO NOT USE THE SUBMIT BUTTON AT THE TOP OF THE APPLICATION, you should also save your completed application.
  3. Make sure your name is on any/all attachments.
  4. Sign and date your application where indicated, by providing your name and birth date.
  5. Have your parent/guardian sign your application where indicated, by providing their name and birth date. Your parent/guardian must sign even if you are 18.
  6. Submit your application to brandee.tolner@medford.k12.or.us  by January 27, 4 P.M.  DO NOT submit a paper copy of your application and DO NOT USE THE SUBMIT BUTTON AT THE TOP OF THE APPLICATION. All application submissions are final; once submitted, you may not make changes or substitute a new application.  Late/incomplete applications will not be accepted.
  7. Verify that the College and Career Center has received your application. After you submit your application you will receive an email stating that the application was received. If you do not receive an email back, GO TO THE COLLEGE AND CAREER CENTER.
